Proctology is a branch of medicine which deals with anus & rectum .

A)Fissure in Ano
A simple rip or crack in the epidermis near the anal canal is known as a fissure-in-ano or anal fissure. Constipation and firm stool can also cause this. A skin tear in the anal canal occurs as a result of overstretching of the skin in that area. The skin around the anal canal is extremely thin and delicate.
Symptoms
Patients usually complain of sharp pain that starts with the passage of stool along with burning defecation and bleeding .Pain can last from several minutes to a few hours. As a result, many patients may try to ignore a bowel movements to prevent pain.
Treatment
Although most of anal fissures do not require surgery,they get settled by conservative approach over 6-8 weeks by stools softeners ,consumption of High Fibre diet .Chronic fissures are harder to treat and surgery may be the best option. The goal of surgery is to relax the anal sphincter muscle which reduces pain and spasms, allowing the fissure to heal. Nowadays due to advent of Laser Surgery The use of a diode laser rather than an electroscalpel offers a superior final result because wound healing begins sooner and scars become smoother and more elastic.
B)Piles (Haemorrhoids)
Piles are engorgements of veins in & around anal canal due to filling of blood more than usual ,it can be internal or external in nature. Because of the enlarged tissues, they normally come out of the anal area . Also most common cause is constipation ,straining while passing urine ,heavy weight lifting .
Symptoms
Most of People don’t have any symptoms of piles,it is said that around 50 % of India’s Population may have piles by the time they reach 50 but only a few show symptoms like bleeding etc .
Larger piles may cause a mucous discharge, some pain, irritation and itching. The discharge may irritate the skin around the anus.Piles can be felt outside the back passage. Often, it is possible to pushed it back up after you have been to the toilet. However, more severe piles remain permanently prolapsed and cannot be pushed back up
Treatment
Approach to Piles treatment usually depends upon grade of Piles .Grade 1 & 2 is usually managed conservatively with Sclerotherapy and Rubber Band ligation .Whereas Grade 3 & 4 requires surgical intervention like Laser Haemorrhoidoplasty or Milligan -Morgan open Haemorrhoidectomy because the prolapsed piles causes discomfort and bleeding .The main advantage of the laser technique is that it causes less pain, if the patients undergo laser surgery, they will heal quickly and be able to return to work as soon as possible.
C)Fistula in Ano
An Fistula-in-ano is a term used to describe an irregular/abnormal tract between Anal canal & perianal skin .Common cause of Fistula in ano is Constipation ,Not maintaining personal hygiene of the Genital area leading to infection of Cryptoglandular glands (Which maintains moisture in Anal Canal ) ultimately leading to perianal abcess & Fistula in ano .
Symptoms
Patient usually complains of Pus discharge from Perianal region with no pain ,usually its chronic in nature
Treatment
It usually depends upon type of Fistula ,and it demands Individualize approach for each patient depending upon cause .We usually undertake an Hybrid approach involving excision of Fistulous tract with scooping of infected anal gland,curettage and Laser ablation of tract .It gives best results to patient with very less of recurrences.
